Job Summary:



Under the supervision of the Director, the  School Nurse provides
assistance in the health screening for sophomore students as well as
students who will participate in JEEP and Praxis.



 Duties and Responsibilities:



1.       Reception and Front Desk Function:

·      Manages shift in Front desk reception and Triage of patients

·      Directs patient flow and provides queueing, as needed

·      Performs as a second response team, as needed



2.   Administrative Support

   - Provides assistance and informs the health screening physicians  on
   the flow of health screening consultation, referrals to OGC and Placement
   Office
   - Prepares the forms and schedule of  students for health screening
   - Facilitates setmore application and oversees the scheduling of
   students for health screening
   - Prepares order for checks  and request  for payment of the physicians
   in the health screening programs and the Instructor of health services
   fitness program



3.       Medical Service Support

·         Takes vital signs, height, weight and BMI of  students for Health
screening

·         Assists the health screening physician during consultation

·        Coordinates with OSCI regarding  schedule of JEEP and Praxis field
work

·        Assists in the screening of medical record of incoming freshman
and checks the complete of the same.



4.       Health Education and Documentation Support

   - Types reports for the weekly census of health screening program of the
   office (sophomore, JEEP and PRAXIS).
   - Prepares Monthly activities and its annual report, includes
   documentations and compilation of annual activities
   - Ensures  updated activity board/  health information board
   - Assists in the activities of the Comprehensive Anti-Drug Abuse Program.



5.       Performs other duties in the clinic, as may be assigned by the
School Physician



*Qualifications:*
1. Registered Nurse
2.  With two years professional nursing experience, one year of which must
have been

     in a school setting

3.  Excellent frontline communication and interpersonal skills

4.  Proficient in MS Office applications
