SUBJECT :       CHEST X-RAY AND ANNUAL PHYSICAL EXAM (APE)
                For FY 2008-2009

The Health Care Plan provided by the University includes an Annual Physical
Examination (APE).  The APE consists of the following: chest x-ray, complete
blood count (CBC), urinalysis, fecalysis, electrocardiogram (ECG) (for 35 years
and up), papsmear (females 35 years and up), and physical medical examination.

For everyone's convenience, the Personnel Office has arranged for the APE to be
conducted at specified venues within the campus.  Employees need to register
and undergo all tests at venues indicated below before proceeding to the x-ray
van.  The van will be parked near the APE venue. Tests will be conducted
between 8:00am to 12:00nn and from 1:00 to 4:00pm at the following dates:


HIGH SCHOOL
Tuesday and Wednesday
February 10 and 11
HS Admin Bldg.
APE:  Old OSA, G/F, Alumni Room


APS MAKATI
Thursday and Friday (12:noon to 6:00 pm)

In addition to this, Pfizer offered a blood chemistry screening (total
cholesterol & fasting blood sugar) free of charge.  Results will be immediately
given and those with findings can avail free consultation and prescription of
appropriate medication through Pfizer invited Physicians who will be with them.
These are their schedules: HS (02-10-2009), GS (02-16-2009) and LS
(02-18-2009).

Fasting for 8 hours is a requirement for blood screening of (total cholesterol
& FBS) and 2 hours for RBS (random blood sugar).  Blood specimen used for the
CBC (part of our APE) will also be used for these tests.

Those who are unable to have their chest x-rays taken on their unit's scheduled
dates and those with incomplete laboratory procedures, can avail of the APE at
the following Clinics only:

Those who availed of the exams but failed to complete these have until March
31, 2009 to do so at these clinics. Those who were unable to undergo any exam
have until March 31, 2009 to do so at these clinics. No APE is allowed
off-campus while on-campus APE is on-going.
	
APE results will be available for pick-up 15 working days from last day of APE
of the unit at the Personnel Office.

Employees who are pregnant and those who have had their x-ray in the past six
months should not have their x-rays taken during their APE.  Recent hires who
have just had their pre-employment exams need not participate in this APE
activity.
