MEMO TO:	All Employee-Members of the Ateneo Health Care Plan (MAXICARE)

SUBJECT :	ANNUAL PHYSICAL EXAMINATION (APE), FY 2002-2003


You are invited to avail of the FREE Annual Physical Examination (APE), of our 
health care plan with Maxicare.

The APE consists of the following: chest-xray, complete blood count (CBC), 
urinalysis, fecalysis, ECG (35 yrs. old and up), Pap smear (females 35 yrs. old 
and up), and medical exam.

To accommodate those who wish to avail of the APE, HealthDev and Personnel 
Office worked out a chest x-ray schedule  (see schedule below).  For those who 
may not be able to follow their unit’s schedule, you may still have the chest x-
ray taken on any of the other dates scheduled on campus.

Those with x-ray finding will be given priority to the rest of the APE 
procedures.  The rest will follow the schedule from February 2003 to June 2003 
grouped according to birthmonths:

*To be taken within a single day:
	CBC, urinalysis, fecalysis		8-10 am only
	Medical examination			9-12 am and 1-5 pm

Employee-Members in the APS units in Makati will have their APE using the 
Maxicare facilities in Makati clinic, in a separate schedule.

Those without any chest x-ray data will not be scheduled for the rest of the 
APE procedures. Those who had their chest x-ray taken within the last six 
months and on maternity cases are exempted. If anyone misses out on any of the 
x-ray dates scheduled on campus, his/her chest x-ray may be taken off-campus 
with the proper referral from HealthDev and subject to payment of a fee of P 
125.00.
